Aurora Community Health is designed to help those in need overcome social factors of pertaining to health within their community and the systemic causes that lead to health inequities. We foster hospital-community partnerships to build a culture of wellbeing by providing clinical and non-clinical approaches for improving health, preventing disease and reducing disparities. Together we address social, behavioral, environmental, economic and medical determinants of health through collaboration and education.
Our case management process consists of 5 parts: assessment, treatment planning, linking, advocacy & monitoring to achieve optimal health and functional capability.
