Our transitional care program helps patients transition from hospital to home, with a focus on medication management, fall risk and arranging follow-up care. We will build a care schedule that meets your requirements up to 24 hours/day, 365 days/year.
It all begins with our complimentary in-home assessment. One of our Care Coordinators will meet with you and your family to gain a full understanding of your medical, personal, and household needs. From there, a detailed care plan can be developed to include all of our included services.
Your Resident Service Plan:
We encourage family's to get involved and take control of your care plan. During your visit with us and before you decide to start services, take a close look at the care plan. It's important to see the service plan, co-write, review and discuss any updates to it.
A care plan is developed from the start, that serves to keep everyone involved in your care – the nursing staff and our care specialist on the same page. We reassess if a change occurs every 30 days and every 3 months.
