Family First Homecare will assist you or your love one as they transition from one level of care to another. Our care specialist are involved with patients who may be discharging from the hospital to home or home based community and are in need of assistance while they heal.
This period carries a high risk of medical errors and adverse events due to fragmented communication, or unsafe hand off due to the need to integrate between the hospital organization and home based care providers.
Re-hospitalizations
Approximately one in five hospitalized patients are readmitted within 30 days and a third of patients are readmitted within 90 days. Our home based care program includes 24- hour support or intermittent assistance for frail older adults, patients with disabilities or those in need of prolonged post- hospitalization care. Our care specialist will provide an effective, safe and appropriate continuation of support.
Family First Homecare's overarching goal is to reduce unnecessary hospitalizations and the cost of custodial care. Our home based care and assistance focuses the majority of services toward the chronically ill or functionally frail patient who may be at a higher risk for patient safety events such as medication errors and falls.
